It would be good to be able to set a default milestone to something other than the milestone at the top of the list.

Just one minor issue: if the default milestone isn't set yet (or the default is deleted) then on /admin/tickets/fields all the radio buttons are selected. The other milestone-specific management page at /tickets/milestones works just fine.

There's a problem marking a new milestone as default on the Field Management screen under Tracker admin: http://screencast.com/t/IukTb9Z18w

When the new milestone is marked as default, the prior default radio button isn't cleared. You can save it this way, and end up with two "defaults".
