#653 tickets: need "add attachment" link on tickets create page, display page

When I create a ticket, I should be able to add an attachment at the time of creation. This should be an "add file" widget consistent with file uploading on the rest of the site, as the last item on the create ticket page.
Similarly, on the display page, I should have the ability to add an attachment when I click-edit the ticket metadata field (below tags).
