#674 discussion: don't let people create a topic with no subject, if they send email use "(no subject)"

From the web interface, I should not be allowed to create a new discussion topic without specifying a title. When I try to edit/create a discussion item without a title it should give an error status message "You need to provide a title for your message"
If there are records that don't have a title (for instance, if they were created via email), those topics should appear with the title "(no subject)"
