When separate ticket edits and comments happen in close order together (I don't remember how to trigger it exactly) then they can be combine into one comment and notification may not go out. We should always keep them separate if they are separate actions.

e.g. https://sourceforge.net/p/allura/tickets/4968/#95f7/2aa4

MAILBOX_QUIESCENT might be a relevant factor, although its theoretically disabled.
