#89 custom fields edit page confusing

the edit page shows an empty custom field on first display, but after that you need to click add new field. We should be consistent, nate suggests always showing a blank one.

This conflicts with [#91] which says the submission should be rejected if there's a custom field with a blank label in the form. If I put in the fix suggested above, there will always be at least one custom field on the form with a blank label. Instead of showing a blank custom field if none have been created, I will show a "No custom fields have been created" message so it's consistent.
